Civitas Solutions, Inc., which markets its services nationally as The MENTOR Network, is a national network of local health and human services providers in 36 states offering an array of quality, community-based services to adults and children with intellectual and developmental disabilities, brain and spinal cord injuries and other catastrophic injuries and illnesses, and to youth with emotional, behavioral and medically complex challenges, as well as their families.

Founded in 1980, Civitas Solutions has grown from a single residential service provider to a national organization providing services and supports in 36 states. Our depth of experience coupled with our ability to offer clinical resources and share best practices across the organization has made us a preferred provider for a diverse group of payors and referral sources across the country. Our community-based services are a high-quality, cost-effective alternative to institutional settings. As of September 30, 2016, we operated in 35 states, serving more than 11,500 clients in residential settings and more than 16,500 clients in non-residential settings.


Human Services

Civitas Solutions’ Human Services segment consists of home- and community-based services to adults and children with intellectual and developmental disabilities (I/DD), to at-risk youth (ARY) with emotional, behavioral and medically-complex challenges, and their families, and to elders in need of support (ADH). Services for individuals with intellectual and developmental disabilities include host homes and community-based residences, as well as day programs and vocational services. Our services for at-risk youth and their families include foster care and periodic family preservation services. As of September 30, 2016, we provided human services to approximately 26,000 individuals.

The delivery of services to individuals with intellectual and developmental disabilities (I/DD) is the largest portion of our Human Services segment. Our I/DD programs include both residential and non-residential services. Our residential services for adults and children with intellectual and developmental disabilities are provided in a range of community-based settings, including small group homes, Intermediate Care Facilities (ICF/MR), host homes and in-home settings. Our non-residential offerings include day programs, vocational services, case management, crisis intervention and hourly support. As of September 30, 2016, we provided I/DD services to approximately 17,600 clients in 21 states.

Civitas Solutions home- and community-based programs for at-risk youth (ARY) and their families include therapeutic and medically complex foster care, family preservation services, school-based services, juvenile offender programs, early intervention, and adoption services. Our individualized approach allows us to work with an ever-changing client population that is as diverse demographically as it is in type and severity of condition. As of September 30, 2016, we provided ARY services to approximately 5,600 children, adolescents and their families in 9 states.

Civitas Solutions' Adult Day Health (ADH) segment serves elders and other adults with complex medical or rehabilitative needs through day programs that enable them to remain in their homes as an alternative to higher levels of care such as nursing facilities. Our adult day health facilities provide center-based services to approximately 3,000 adults in two states, Massachussetts and Maryland. Services include medical oversight, recreational activities, nutrition supports and transportation.

Specialty Rehabilitation Services (SRS)

Civitas Solutions’ SRS segment delivers health care and community-based rehabilitation services to individuals who have experienced brain and spinal cord injuries, as well as to individuals with other catastrophic injuries and illnesses. Services range from sub-acute healthcare for individuals with intensive medical needs to day treatment, and include neurorehabilitation, neurobehavioral rehabilitation, skilled nursing, physical, occupational and speech therapies, supported living, outpatient treatment, and pre-vocational services. SRS programs are delivered primarily through specialized community facilities, small group homes, in-home and non-residential settings. As of September 30, 2016, our SRS operations provided services in 26 states and served approximately 1,800 clients nationally.

CareMeridian CareMeridian provides a continuum of high-quality, cost-effective, post-acute care and rehabilitation options to people of all ages with brain, spinal cord and other life-altering injuries and illnesses. With program locations throughout the western United States, CareMeridian offers a distinctive service delivery model, merging an experienced and skilled health care and rehabilitation team with state-of-the-art technology in community-based program settings.

NeuroRestorative NeuroRestorative provides post-acute rehabilitation services for people of all ages with brain and spinal cord injuries and other neurological challenges. In a variety of locations and settings, NeuroRestorative offers a continuum of care and rehabilitation options, including specialized services for children, adolescents, and active duty service members and Veterans.
