Civitas Solutions, Inc., which markets its services nationally as The MENTOR Network, is a national network of local health and human services providers offering an array of quality, community-based services to adults and children with intellectual and developmental disabilities, brain and spinal cord injuries and other catastrophic injuries and illnesses, and to youth with emotional, behavioral and medically complex challenges, as well as their families.

Founded in 1980, Civitas Solutions has grown from a single residential service provider to a national organization providing services and supports in more than 30 states. Our depth of experience coupled with our ability to offer clinical resources and share best practices across the organization has made us a preferred provider for a diverse group of payors and referral sources across the country. Our community-based services are a high-quality, cost-effective alternative to institutional settings. As of June 30, 2018, we serve more than 12,500 clients in residential settings and more than 19,000 clients in non-residential settings.

Specialty Rehabilitation Services (SRS)

Civitas Solutions Specialty Rehabilitation Services (SRS) service line delivers health care and community-based rehabilitation services to individuals who have experienced brain and spinal cord injuries, as well as to individuals with other catastrophic injuries and illnesses. Services range from sub-acute healthcare for individuals with intensive medical needs to day treatment, including neurorehabilitation; neurobehavioral rehabilitation, such as counseling and substance abuse treatment; skilled nursing; physical, occupational and speech therapies; supported living; outpatient treatment; pre-vocational services; youth education services; and in-home personal supports to assist participants with daily living. SRS programs are delivered primarily through specialized community facilities, small residences, in-home and non-residential settings.

Services in our Specialty Rehabilitation Services service line are delivered by our partners NeuroRestorative, CareMeridian and Mentis.


As of June 30, 2018, we provided SRS services to approximately 2,200 clients in 25 states.

Children & Family Services (CFS)

Civitas Solutions home- and community-based programs for youth at risk and their families offer services tailored to the needs of children with behavioral health challenges, complex medical issues and other needs. Our individualized approach allows us to work with an ever-changing population that is as diverse demographically as it is in type and severity of challenge. Our continuum of family-based personal supports include therapeutic and medically complex foster care; family preservation services, including counseling and crisis management; school-based services encompassing academic instruction, tutoring, and therapeutic and vocational supports; juvenile offender programs; early intervention and crisis intervention; adoption services; and case management to ensure children receive all needed social, medical, educational and other services.

As of June 30, 2018, Civitas Solutions Children & Family Services (CFS) service line served approximately 6,000 children, adolescents and their families in 9 states.

ADH Services

Civitas Solutions Adult Day Health (ADH) service line serves elders and other adults with complex medical or rehabilitative needs as an alternative to higher levels of care, such as nursing facilities. Our day programs ensure that participants have daily support in managing a range of conditions, including dementia, rehabilitative challenges following stroke or injury, and chronic illnesses such as diabetes. Services are tailored to each person’s unique needs and include nursing care, social activities, nutritional supports, behavioral health services, rehabilitation and transportation.

As of June 30, 2018, 0ur adult day health facilities provide center-based services to approximately 5,100 adults in three states: Massachussetts, New Jersey and Maryland.

Community Support Services (CSS)

Civitas Solutions Community Support Services (CSS) delivers services to individuals with intellectual and developmental disabilities, offering both residential and non-residential supports. Our residential services for adults and children are provided in a range of community-based settings, including small residences, Intermediate Care Facilities (ICF/MR), host homes and in-home settings. Our non-residential offerings include day programs; vocational and job training services; behavioral health services, including counseling and crisis intervention; hourly personal supports ranging from independent living to relief services; and case management to ensure individuals receive all needed social, residential and other supports.

As of June 30, 2018, we provided Community Support Services (CSS) to approximately 18,300 clients in 21 states.
