Civitas Solutions Adult Day Health (ADH) service line serves elders and other adults with complex medical or rehabilitative needs as an alternative to higher levels of care, such as nursing facilities.
Our day programs ensure that participants have daily support in managing a range of conditions, including dementia, rehabilitative challenges following stroke or injury, and chronic illnesses such as diabetes.
Services are tailored to each person’s unique needs and include nursing care, social activities, nutritional supports, behavioral health services, rehabilitation and transportation.
